LHSAA Physical Form*

• Physicals must be performed by a licensed medical professional.
• All athletes must have a completed physical before participation of any kind.
• Physical is valid for one calendar year from the date of examination.

CMS Athletic Packet* 

• The Athletic Packet must be completed for the athlete to participate.
• Be sure to sign, initial, or date in all areas.
• There are areas for the parent/guardian to sign as well as for the student/athlete to sign.
• Please look over the packet carefully before turning it in.
